Planning bedside support: a family handover checklist
Get responsibilities, communication and the existing care plan clear before a bedside shift begins.
Begin with the existing care plan
Bed rest can have different causes, so support should follow the treating clinician’s instructions. Bring the discharge or care plan to the assessment. Ask the clinician to clarify restrictions and which tasks require a qualified nurse. This guide does not give turning schedules, treatment or lifting instructions.
Make responsibilities easy to understand
Agree which person handles meals, hygiene support, laundry, supplies and contact with the clinician. Discuss privacy, dignity and the patient’s preferences. Ask what equipment or training the care provider requires, and confirm charges for the agreed duties.
Make every shift handover consistent
Choose a family contact, agree an update time, and keep essential instructions available at home. Record practical changes in the daily routine without changing medicines or clinical instructions independently. Make a plan for staff absence and clarify who arranges urgent medical help.
